Public Protection

In order to ensure our photo ID card system contains accurate and truthful information, and to reduce fraudulent, terrorist, or other criminal misuse of your photo ID card, we require the following limited information: Full name, address, telephone number, drivers licence, or other acceptable means of identification. We will take your photograph when you register. We reserve the right to verify information by all means available. Such means could be telephone, written requests, and may include the use of government agencies, credit bureaus, or any other parties available to help ensure accuracy and truthfulness.

Your Personal Information Protection

We at DGS Canada must have your consent before collecting any personal information. Any and all forms you voluntarily complete require your signature before we accept or use such forms for any purpose. All such signed forms or documents are taken specifically for the purpose of individual verification, to maintain records of your training and certification, to allow us to provide course comments, photograph, and name only for our marketing purposes, and to allow us to contact you for purposes of re-certification, re-training or for notification of new training programs. In addition we may receive employer requests for certification confirmation. DGS Canada may not sell your personal information to any party. You have the right to review your file material, providing you notify us in writing. Within 48 hours from receipt of notice, we will arrange viewing. Copies of registration or comment documents may be provided for a fee of $25.00 per document. Exam results can be confirmed verbally only, because of business volume and confidentiality. We are not responsible for missing documents of any type whatsoever.

Any requests for information, from outside parties, with respect to your file, will initiate a written or telephone response to the last address provided by you, advising you of the nature of the request. In the event you return your photo ID card and certificate indicating you are no longer operating industrial equipment, we will seal all file records immediately, except transaction information duly required by Federal or Provincial laws.
